- Care Coordinator- Geriatric Care Manager
Description
Position Overview:
As a Care Coordinator, you will play a vital role in supporting our members by identifying their evolving needs and creating personalized care and wellness plans. You'll work closely with members, families, healthcare providers, and community resources to ensure seamless care and optimal well-being. You will also work closely with our 8-member Care Coordination team virtually and sometimes in person, to support our mission. You will work remotely from your home office in the Columbus region, while also visiting members in their homes throughout Central Ohio. There will be some oversite to members in the Dayton, and/or Northeast Ohio regions.
What You’ll Do:
Manage a caseload of members, conducting assessments and developing individualized care plans.
Coordinate services, provide oversight to care plans.
Serve as a trusted advocate, educating members and their families about available resources and benefits.
Work collaboratively with service providers, and other key stakeholders.
Participate in wellness initiatives, community outreach, and sales support activities.
Maintain accurate documentation of all member interactions and care plans.
Be part of our 24/7 on-call rotation, (typically once every 8 weeks) offering support.
This is a 40 hour per week position, but would consider 30 hours per week

Requirements
Bachelor's degree
Driver's License
work with older adults
documentation in electronic health record
PTA, COTA, Social Work, Nursing backgrounds preferred
