**Greater Lynn Senior Services has 2 openings for Geriatric Support Services Coordinator - 1 Part Time and 1 Full Time**
The Geriatric Support Services Manager coordinates the Senior Care Options multidisciplinary health care team. Responsible for all aspects of support service case management for elder client caseload, including needs assessment, service plan development, service authorization, and monitoring service needs of clients.
Requirements
Qualifications: Minimum Bachelor’s Degree with 2 years elder care case management, at least one year must be in a setting that provides health care to persons over 65 years old. Applicants must speak Spanish and be knowledgeable of elder care network. It is a requirement of the GSSC/Spanish speaking position to have reliable transportation and valid driver’s license or ability to make home visits. Applicants must actively participate in interdisciplinary team environment. Interested candidates must have excellent communication, documentation skills and an interest in marketing services.
Hours: Full Time – 40 hours per week (Monday-Friday) or Part Time - 20 hours per week
