Care Coordinator Ad :

Sumner Mental Health Center, a Certified Community Behavioral Health Center, is seeking a Care Coordinator to assist individuals and their families per CCBHC criteria, providing support in connecting and communicating with other providers, conducting screenings and data collection to determine physical and behavioral health needs, and in acting as an ongoing information resource throughout the treatment process. This position requires someone 21 years of age or older with a BA or with an Associates Degree combined with social services work, prior experience with documentation in an electronic health record, including the collection and inputting of data, ability to both work one-on-one with individuals and families, including those in vulnerable populations, as well as collaboratively with outside providers and community entities, strong verbal and written communication skills, organized, able to closely work with an individual's treatment team, research and advocacy skills, cultural competence, and understands the importance of confidentiality. Preference for experience working in community mental health or similar social services and/or coordinating physical/behavioral health treatment.
