Job Description
Tempus Unlimited, Inc. is a nonprofit organization that provides community-based services to empower children and adults with disabilities to live as independently as possible in the least restrictive environment. The agency, through its programs and services, encourages the inclusion of people with disabilities into the mainstream of society, including social, recreational, family and work activities.
The Adult Foster Care (AFC) Supervisor provides support, training and coaching to AFC caregivers and program members. They play a key role in ensuring quality care by offering support to Care Managers, conducting site visits, and providing oversight for the Community Services Adult Foster Care program. The AFC program itself offers a home-based care setting for individuals who need assistance with daily activities.
Essential Functions
- Provide direct support to the care managers/nurses in the community
- Conduct site visits to consumer homes and produce proper quality feedback /reports according to manager's guidance
- Offer input to annual performance evaluations
- Provide monitoring and oversight for efficiencies with all referrals in AFC
- Lead and assist with program team meeting
- Support and train staff during the orientation processes or initial planning meeting to insure proper efficiencies in services
- Support Community Living Program Manager as assigned
- Monitor LTSS process to ensure accuracy and timely submittals
- Assign caseloads
- Provide supervision to nurses and care managers
- Handle member or caregiver complaints
- Support new program development and implementation with program
- Additional responsibilities as assigned
Competencies
- Three or more years of recent experience working with people who are elderly or disabled
- Ability to supervise and direct staff
- Excellent organizational skills
- Attention to accuracy and detail
- Strong interpersonal skills telephone skills
- Excellent oral and written communication
- Good computer and analytical skills
- Innovative thinking
- Ability to maintain confidentiality
- Perform all other duties as may be required
Preferred Experience
- Supervisory experience preferred
Required Education
- Bachelor's degree required
- A Human Services degree is preferred but can be substituted with a related degree with field experience
Work Environment
- This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines
Physical Demands
- The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job
- While performing the duties of this job, the employee is regularly required to talk and/or hear. The employee is frequently required to sit; stand; walk; use hands to finger, handle or feel; and reach with hands and arms

- Travel is required for this position. Must have a valid driver's license and reliable transportation
